GLP-1 Receptor Agonist (GLP-1 RA) Improves Men's Health

The recent hype of use of GLP-1 RA in weight loss can be beneficial in male’s sexual function and testosterone levels. Further to effective weight reduction, GLP-1 RA is postulated to enhance obese male reproductive function.

Obesity is a chronic medical condition

The cause of raised BMI (body mass index) is multifactorial with genetic susceptibility, potential concurrent medical conditions and environmental influences such as person’s lifestyle and cultural behaviors and socio-economic status.

GLP-1 RA mimics the body's natural way of responding and metabolizing blood glucose, and affects various organs of the body.

Common GLP-1 RAs seen in the market including Semaglutide, Liraglutide and Dulaglutide. Interestingly, these medications are also used in the management of diabetes though they are used at a different dosage in the management of obesity.

Obesity Results in Male Hormone Dysregulation

Uncontrolled weight can reduce the chances of conceiving in men.

The hypothalamus and the pituitary gland of the brain play important roles in regulating male hormones. In obesity, this brain-reproductive organ regulation can be disrupted, leading to inadequate production of male hormones such as testosterone.

To make things worse, excessive adipose (fat) tissues present in obesity convert testosterone into estradiol through a process known as aromatization. This results in even lesser testosterone in the blood circulation.

Those with obesity also has lesser sex hormone-binding globulin (SHBG) that is paramount in binding sex hormones and ferrying them around the body. Low SHBG causes a state of greater effect of estradiol, and lesser effect of testosterone in the body.

Furthermore, other hormones such as growth-like factor, inhibin B are reduced in obesity leading to lesser testosterone production by the testis.

The pro-inflammatory state of obesity leads to a general less conducive environment for one to produce healthy testosterone hormones.

Positive Effect of GLP-1 RA on Male Hormones

GLP-1 receptors can be found in male reproductive organs. They play an important role in sperm production.

GLP-1 RA increases testosterone levels (both total testosterone and free testosterone) in obese men. In addition, there are significant rise in levels of brain hormones (known as gonadotropins) which subsequently facilitate the downwards cascading effect on the testis to produce testosterone and sperm.

Indirectly, due to weight loss, better glucose control and improved insulin sensitivity following the use of GLP-1 RA in obesity, one may notice higher male hormones.

Effect of GLP-1 RA on Erectile Dysfunction (ED)

The actual role of GLP-1 RA in obese men experiencing erectile dysfunction is not fully understood.

ED is a condition closely related to age, the neuro-blood circulation function around the penis, and hormonal control. Many of those suffering with obesity have concurrent metabolic conditions such as diabetes and ED.

GLP-1 Agonist is believed to have anti-inflammatory effect, protecting and preserving the nerves and blood vessels around the penis by modulating one's glucose and lipid metabolism resulting in improvement in ED.

Effect of GLP-1 RA on semen and sperm quality

Human's body shows presence of GLP-1 receptors over the sertoli and leydig cells of the testis- both cells regulates the process of sperm production. GLP-1 RA has a positive effect on both sertoli and leydig cells, resulting in better sperm production, sperm energy metabolism and sperm movement.

GLP-1 RA can improve the sperm quality, sperm amount and sperm motility in obese male.

Impact of GLP-1 RA on Cardiovascular Risk of Men

Use of GLP-1 RA is associated with a significant reduction of major adverse cardiovascular events such as stroke, heart attack and death. Also, GLP-1 RA is known to improve patients’ outcomes who has concurrent heart failure and chronic kidney disease.

By losing weight, improving the body’s glucose control and cholesterol levels, studies have proven that GLP-1 RA improves cardiovascular morbidity and mortality.
