What is delayed ejaculation?

Delayed ejaculation, or impaired ejaculation, occurs when a man is consistently unable to get an orgasm and ejaculate even with sufficient sexual stimulation and arousal. Up to 20-30% of sexually active men experience premature ejaculation. While delayed ejaculation can happen from time to time, for some men, it is a lifelong issue.

What are the symptoms of delayed ejaculation?

You may have impaired ejaculation if you take more than 30 minutes of sexual stimulation and activity to ejaculate. If you cannot ejaculate at all, you have a condition known as anejaculation.

Though rare, sometimes delayed ejaculation can be a sign of a worsening health condition such as diabetes or heart disease.

  • Psychological causes e.g. traumatic experience, depression, anxiety, religious or cultural taboos
  • Relationship problems e.g. poor communication, relationship stress, anger
  • Sexual fantasies i.e. some men are able to ejaculate while masturbating but not with a partner
  • Medications and substances e.g. antidepressants, antipsychotics, medications for high blood pressure, diuretics, alcohol, morphine
  • Physical causes e.g. damage to the nerves in your pelvis or spine, prostate surgery, stroke, low testosterone levels, low levels of thyroid hormone, urinary and/or prostate infections, birth defects, retrograde ejaculation

Note: A temporary ejaculation problem can lead to depression and anxiety. This can ultimately result in recurrent DE episodes, even when the existing physical cause has been tackled.

How is delayed ejaculation diagnosed?

To make the correct diagnosis, your doctor will need to have a detailed explanation of your symptoms. Your doctor will also conduct a physical examination. If an underlying health condition is identified as the cause of your delayed ejaculation symptoms, treating that particular condition should ease your delayed ejaculation symptoms. Evaluating the reaction of your penis to a vibrator may help determine whether the issue is physical or psychological.

How is delayed ejaculation treated in Singapore?

Treatment usually starts with treating any underlying health conditions. If you are on medication, your doctor will advise you to either stop or change any medications that may be causing your delayed ejaculation symptoms.
